Study Tools and Worksheets

Study Pocket Card – This pocket card is designed as a quick-reference tool for staff involved in direct trial-related patient care. It contains inclusion/exclusion criteria, a table of procedures, important trial contact numbers, etc. Additional cards are available for resupply.

Screening Log – Allows you to list all patients consented and screened, regardless of enrollment status. Use this log to assign the Subject Number and store completed logs in your Study Manual.

Screening Worksheet – Allows you to confirm each patient’s eligibility and to collect the required data needed for randomization.

Confidential Patient Information – Complete the form for all patients at enrollment and enter the data into the iCRF. The information collected is critical for patient follow-up.

Visit Record – Use one per patient to ensure all required trial procedures are performed from screening through follow-up.

Infusion Visit Worksheet (With Labs) – Use to ensure all required data are obtained for each visit requiring laboratory specimens. This may be used as a source document.

Infusion Visit Worksheet (No Labs) – Use to ensure all required data are obtained for each visit not requiring laboratory specimens. This may be used as a source document.

Vitamin Accountability Log – Use to record dates vitamins are dispensed to or returned from patients.
